SECTION 1 โ€” Priority Map & Quick Foundation
Topic Priority Why It’s Tested
TSHโ€“fTโ‚„ feedback & interpretation patterns HIGH The #1 thyroid concept on the exam โ€” every disorder is a TSH/fTโ‚„ pattern
Hypothyroidism (Hashimoto, central, subclinical) HIGH Most common disorder; anti-TPO and pattern recognition heavily tested
Hyperthyroidism / Graves & thyrotoxicosis HIGH TSI/TRAb, T3 toxicosis, thyroid storm โ€” classic vignettes
Free vs total hormone & binding-protein effects HIGH Pregnancy/estrogen โ†‘TBG traps; free hormone hypothesis
TSH assay generations & functional sensitivity MEDIUM 3rd-gen = 0.01 mIU/L; CV<20% definition
Hormone synthesis (NIS, TPO, Tg, organification) MEDIUM Method/defect questions (perchlorate discharge, Pendrin)
Thyroglobulin & calcitonin as tumor markers MEDIUM Tg (differentiated cancer) vs calcitonin (medullary) โ€” don’t mix up
Drugs (amiodarone, lithium, phenytoin, propranolol) MEDIUM Amiodarone 37% iodine; interference patterns
Embryology & anatomy LOW Foramen cecum, C-cell origin โ€” occasional recall items
๐ŸŽฏ Where to spend your time
Master the TSH/fTโ‚„ interpretation grid first โ€” it unlocks half the chapter. Then lock in Hashimoto vs Graves antibodies and the free vs total binding-protein traps. Skim embryology and deep synthesis biochemistry; know only the names that map to a defect test (NIS, TPO, Pendrin, perchlorate).
Quick Foundation (zero-to-clear)
  • The thyroid makes two hormones: Tโ‚„ (thyroxine, the reservoir) and Tโ‚ƒ (triiodothyronine, the active one). Most active Tโ‚ƒ is made outside the gland by removing one iodine from Tโ‚„.
  • The pituitary releases TSH to tell the thyroid “make more.” Hormone levels feed back to shut TSH off โ€” a thermostat.
  • TSH moves opposite to thyroid hormone in primary disease: gland fails โ†’ hormone low โ†’ TSH high (hypothyroid); gland overactive โ†’ hormone high โ†’ TSH low (hyperthyroid).
  • Over 99% of hormone rides on carrier proteins (mainly TBG). Only the tiny free fraction is active โ€” so anything that changes carrier proteins fakes out total measurements.
  • Iodine is the raw material. Too little โ†’ goiter; sudden too much โ†’ temporary shutdown (Wolff-Chaikoff).
  • Most thyroid disease is autoimmune: Hashimoto (destroys โ†’ hypo) and Graves (stimulates โ†’ hyper).
๐Ÿง  Analogy
Think of TSH as a thermostat and thyroid hormone as room temperature. If the room is cold (low hormone), the thermostat cranks up (high TSH). If the room is hot (high hormone), the thermostat shuts off (low TSH). When the thermostat itself is broken (pituitary problem), the readings stop making sense โ€” that’s central disease.
Concept Plain-English Explanation
TSH Pituitary’s “make hormone” signal; the most sensitive single screening test
Tโ‚„ vs Tโ‚ƒ Tโ‚„ = stored/inactive bulk; Tโ‚ƒ = active worker made mostly by deiodinating Tโ‚„
Free vs Total Free = unbound, active; Total = free + protein-bound (skewed by TBG changes)
Primary vs Central Primary = gland problem (TSH compensates); Central = pituitary/hypothalamus problem (TSH inappropriate)
Autoimmune Hashimoto antibodies destroy; Graves antibodies stimulate

SECTION 2 โ€” Core Analytes & Methods
TSH (Thyroid-Stimulating Hormone / Thyrotropin)
Feature Details
Definition Anterior pituitary glycoprotein; ฮฑ-subunit shared with hCG, FSH, LH; unique ฮฒ-subunit confers specificity
Normal range ~0.4โ€“4.0 mIU/L (assay/population dependent)
Mechanism of change Rises when thyroid hormone falls (loss of negative feedback); falls when hormone is high. Log-linear inverse relationship with fTโ‚„
Clinical significance Single most sensitive screen for primary thyroid disease; a small fTโ‚„ change causes a large TSH change
Assay method Two-site immunometric (“sandwich”) assay. 3rd generation = functional sensitivity 0.01 mIU/L (CV < 20%)
Key interferences Heterophile/biotin interference; macro-TSH (falsely high); inappropriate in central disease

TSH is the workhorse screen, but it is only valid when the pituitaryโ€“thyroid axis is intact. In central disease TSH can be low, normal, or even mildly high yet biologically inactive โ€” so it cannot stand alone.

๐Ÿงช Pre-analytical โ€” TSH
Serum (SST) or plasma. Diurnal variation: TSH peaks late night/early morning, lowest mid-afternoon โ€” standardize draw time for borderline results. Recent biotin supplements can falsely lower TSH in some immunoassays (hold ~2 days). Stable refrigerated; freeze for delay.
Thyroxine (Tโ‚„) โ€” Total & Free
Feature Details
Tissue source Thyroid follicular cells (only source of Tโ‚„); the main secretory product
Transport ~99.97% protein-bound: TBG (main) > transthyretin (TTR) > albumin; only ~0.03% free
Mechanism of change Total Tโ‚„ tracks both gland output AND binding-protein quantity; free Tโ‚„ tracks true thyroid status
Clinical significance fTโ‚„ confirms/stages disease after abnormal TSH; the storage/reservoir hormone
Assay method fTโ‚„: analog immunoassay (routine) vs equilibrium dialysis / ultrafiltration (gold standard)
Key interferences โ†‘TBG (estrogen/pregnancy) raises total Tโ‚„ with normal free Tโ‚„; FDH falsely raises total (and some free) Tโ‚„
๐Ÿงช Pre-analytical โ€” Tโ‚„
Serum preferred. Estrogen, pregnancy, OCPs raise TBG โ†’ high total Tโ‚„ but normal fTโ‚„ and TSH (euthyroid). Androgens, nephrotic syndrome, malnutrition lower TBG. Heparin can falsely raise fTโ‚„ in vitro by releasing free fatty acids that displace Tโ‚„. Avoid biotin interference.
Triiodothyronine (Tโ‚ƒ)
Feature Details
Source ~20% direct thyroid secretion; ~80% from peripheral 5′-deiodination of Tโ‚„ (D1, D2)
Activity The active hormone โ€” higher affinity for nuclear THR than Tโ‚„
Clinical significance Key for diagnosing T3 toxicosis (suppressed TSH, normal fTโ‚„, high fTโ‚ƒ/tTโ‚ƒ)
Reverse Tโ‚ƒ (rTโ‚ƒ) Inactive; โ†‘ in non-thyroidal illness as D3 shunts Tโ‚„ away from active Tโ‚ƒ
Key interferences Falls in non-thyroidal illness (“low T3 syndrome”) โ€” do NOT order in sick inpatients to screen thyroid
๐Ÿงช Pre-analytical โ€” Tโ‚ƒ
Tโ‚ƒ is the first hormone to fall in non-thyroidal (euthyroid sick) illness โ€” low Tโ‚ƒ in a hospitalized patient usually reflects illness, not thyroid disease. Avoid thyroid testing in acute illness unless clearly indicated.
Thyroglobulin (Tg) & Calcitonin โ€” Tumor Markers
Feature Thyroglobulin (Tg) Calcitonin
Source Follicular cells (colloid scaffold protein) Parafollicular (C) cells โ€” neural crest origin
Marker for Differentiated (papillary/follicular) thyroid cancer follow-up after thyroidectomy Medullary thyroid carcinoma (MTC) diagnosis & monitoring
Key caveat Must report with anti-Tg antibody status โ€” anti-Tg causes false-low Tg Linked to RET proto-oncogene; part of MEN2
Method Immunoassay (ยฑ LC-MS/MS when antibodies present) Immunoassay
๐Ÿงช Pre-analytical โ€” Tg
Always measure anti-Tg antibodies alongside Tg. Endogenous anti-Tg antibodies cause clinically dangerous false-negative Tg results in immunometric assays (a recurrence can be missed). Tg only meaningful after total thyroidectomy/ablation.
Synthesis Machinery (defect-test relevance)
Player Role Exam handle
NIS (SLC5A5) Basolateral pump: iodide into cell against gradient Defect โ†’ no uptake; perchlorate discharge negative
Pendrin (SLC26A4) Apical iodide transport into lumen Pendred syndrome (goiter + deafness)
TPO Iodinates tyrosines + couples to form Tโ‚ƒ/Tโ‚„ (needs Hโ‚‚Oโ‚‚) Target of anti-TPO antibodies; organification defect
Thyroglobulin (Tg) Scaffold for synthesis & colloid storage Also the cancer marker
DUOX1/DUOX2 Generate Hโ‚‚Oโ‚‚ at apical membrane Required for organification

SECTION 3 โ€” Memory Hooks & Pattern Triggers
๐Ÿ”‘ “TSH goes the OPPOSITE way” (primary disease)
Gland fails โ†’ hormone โ†“ โ†’ TSH โ†‘ (hypo). Gland overworks โ†’ hormone โ†‘ โ†’ TSH โ†“ (hyper). If TSH and fTโ‚„ move the same direction โ†’ think central disease, TSHoma, or RTH.
๐Ÿ”‘ Antibody mnemonic โ€” “TPO Trashes, TSI Turbocharges”
Anti-TPO โ†’ Hashimoto (Trashes the gland โ†’ hypo). TSI/TRAb โ†’ Graves (Turbocharges the receptor โ†’ hyper).
๐Ÿ”‘ Tumor markers โ€” “Tg = Thyroid-cancer (differentiated); Calcitonin = C-cells (medullary)”
Both start with the cell of origin. Follicular cell โ†’ Thyroglobulin. C cell โ†’ Calcitonin โ†’ RET.
๐Ÿ”‘ Thyroid storm โ€” “Five Bs”
Block synthesis (thionamide) ยท Block release (iodine, given AFTER thionamide) ยท Block Tโ‚„โ†’Tโ‚ƒ conversion (PTU, propranolol, steroids) ยท Beta-blocker ยท Block enterohepatic circulation (cholestyramine).
Pattern Trigger Table
If you seeโ€ฆ Thinkโ€ฆ Becauseโ€ฆ
High total Tโ‚„, normal TSH, pregnant/on estrogen โ†‘TBG, euthyroid (not hyperthyroid) Estrogen raises binding protein; free hormone & TSH stay normal
High TSH + normal fTโ‚„ Subclinical hypothyroidism Gland compensating; hormone still in range
Low TSH + normal fTโ‚„ + high fTโ‚ƒ T3 toxicosis Excess Tโ‚ƒ suppresses TSH before fTโ‚„ rises
Low fTโ‚„ + low/normal TSH Central hypothyroidism Pituitary not responding to low hormone
High fTโ‚„ + non-suppressed TSH TSHoma or RTH-ฮฒ Inappropriate TSH secretion or receptor resistance
Low Tโ‚ƒ, low/normal everything in a sick inpatient Non-thyroidal illness D3 โ†‘ rTโ‚ƒ; don’t diagnose thyroid disease acutely
Falsely low Tg in cancer follow-up Anti-Tg antibody interference Antibodies block the immunoassay
Commonly Confused โ€” Side by Side
  Hashimoto Graves
Direction Hypothyroid Hyperthyroid
Antibody Anti-TPO (ยฑ anti-Tg) TSI / TRAb (stimulating)
Mechanism T-cellโ€“mediated destruction Antibody stimulates TSH receptor
Clue Painless goiter, fatigue Ophthalmopathy, pretibial myxedema, acropachy
TSH / fTโ‚„ โ†‘TSH / โ†“fTโ‚„ โ†“TSH / โ†‘fTโ‚„
Diagnostic Window โ€” Time-Based Markers
Marker / event Onset Peak Duration Best clinical use
Neonatal TSH surge (cold stress) Within 30 min of birth ~30 min Falls over 24โ€“48 h Time newborn screen โ‰ฅ24โ€“48 h to avoid false positives
Levothyroxine โ†’ steady state Days โ€” ~6 weeks Recheck TSH no sooner than 6 weeks after dose change
Postpartum thyroiditis Weeks after delivery Hyper then hypo phase Within 12 months Transient; usually resolves
Amiodarone effect Weeks โ€” Half-life 40 ยฑ 10 days Thyroid effects persist long after stopping

SECTION 4 โ€” Diagnostic Flowcharts
โ‘  Differential โ€” Fatigue, weight gain, cold intolerance
Step Path
Start Patient: fatigue, weight gain, cold intolerance โ†’ order TSH
โ†’ TSH high โ†’ measure fTโ‚„
โ†’ fTโ‚„ low โ†’ Primary hypothyroidism โ†’ check anti-TPO
โ†’ Anti-TPO positive โ†’ Hashimoto thyroiditis (diagnosis)
โ†’ fTโ‚„ normal (TSH high) โ†’ Subclinical hypothyroidism
โš ๏ธ Common mistake Diagnosing hyperthyroidism from a high total Tโ‚„ in a pregnant woman โ€” that’s โ†‘TBG. Check TSH/fTโ‚„ first.
โ‘ก Differential โ€” Palpitations, weight loss, heat intolerance
Step Path
Start Patient: palpitations, weight loss, tremor โ†’ order TSH
โ†’ TSH low/suppressed โ†’ measure fTโ‚„ and fTโ‚ƒ
โ†’ fTโ‚„ high โ†’ Overt hyperthyroidism โ†’ check TRAb/TSI ยฑ uptake scan
โ†’ TRAb positive + eye signs โ†’ Graves disease
โ†’ fTโ‚„ normal but fTโ‚ƒ high โ†’ T3 toxicosis
โš ๏ธ Common mistake Stopping at a normal fTโ‚„ and calling the patient euthyroid โ€” you missed T3 toxicosis. Always add fTโ‚ƒ when TSH is suppressed but fTโ‚„ is normal.
โ‘ข Timing/Marker Selection โ€” Thyroid cancer follow-up
Step Path
Start Post-thyroidectomy for differentiated cancer โ†’ monitor recurrence
โ†’ Order Thyroglobulin (Tg) + anti-Tg antibody together
โ†’ Anti-Tg negative โ†’ Tg reliable โ†’ rising Tg = recurrence
โ†’ Anti-Tg positive โ†’ immunoassay Tg unreliable (false low) โ†’ use LC-MS/MS / trend antibodies
โ†’ If medullary cancer instead โ†’ use calcitonin (+ RET testing), not Tg
โš ๏ธ Common mistake Trusting a low Tg without checking anti-Tg status โ€” antibody interference can mask recurrence.
โ‘ฃ Method Selection โ€” Free Tโ‚„: analog vs dialysis
Step Path
Start Need free Tโ‚„ result
โ†’ Normal binding proteins, routine case โ†’ analog immunoassay (fast, cheap)
โ†’ Abnormal binding (pregnancy, FDH, severe illness) or discordant result โ†’ equilibrium dialysis / ultrafiltration (gold standard)
โš ๏ธ Common mistake Believing an analog fTโ‚„ in a patient with binding-protein abnormalities โ€” analog assays are still TBG-sensitive; confirm with dialysis.
โ‘ค Synthesis Defect โ€” Perchlorate discharge test
Step Path
Start Goiter + suspected synthesis defect โ†’ give radioiodine, then perchlorate
โ†’ Iodide discharged (>10% release) โ†’ organification defect (TPO / Pendrin problem)
โ†’ No uptake to begin with โ†’ NIS (trapping) defect
โš ๏ธ Common mistake Confusing a trapping (NIS) defect with an organification defect โ€” perchlorate only discharges iodide that was trapped but not organified.

SECTION 5 โ€” 25 High-Yield Facts
# Fact
1 TSH is the single most sensitive screening test for primary thyroid dysfunction.
2 In primary disease, TSH moves opposite to thyroid hormone.
3 Only ~0.03% of Tโ‚„ is free; the rest is bound (mainly to TBG).
4 Most circulating Tโ‚ƒ comes from peripheral deiodination of Tโ‚„ by D1 and D2.
5 Tโ‚ƒ is the active hormone (higher nuclear-receptor affinity than Tโ‚„).
6 Estrogen/pregnancy raise TBG โ†’ high total Tโ‚„ with normal free Tโ‚„ and TSH.
7 Hashimoto thyroiditis is the most common hypothyroidism cause in iodine-sufficient areas.
8 Anti-TPO is the sensitive antibody marker for Hashimoto.
9 Graves disease is the most common cause of thyrotoxicosis in iodine-sufficient areas.
10 TSI / TRAb (stimulating antibodies) cause Graves disease.
11 Subclinical hypothyroidism = high TSH with normal fTโ‚„.
12 Central hypothyroidism = low fTโ‚„ with low/normal/slightly high TSH.
13 T3 toxicosis = suppressed TSH, normal fTโ‚„, elevated fTโ‚ƒ/tTโ‚ƒ.
14 A 3rd-generation TSH assay has functional sensitivity of 0.01 mIU/L.
15 Functional sensitivity = the concentration where assay CV < 20%.
16 Equilibrium dialysis is the gold standard for free Tโ‚„.
17 Thyroglobulin is the tumor marker for differentiated thyroid cancer; always report anti-Tg status.
18 Calcitonin is the marker for medullary thyroid carcinoma (C-cell origin, RET).
19 Papillary carcinoma is the most common, best-prognosis thyroid cancer; anaplastic is the most lethal.
20 Amiodarone is ~37% iodine by weight and can cause hypo- or hyperthyroidism.
21 Lithium inhibits thyroid hormone release โ†’ hypothyroidism.
22 The Wolff-Chaikoff effect = transient synthesis inhibition after an iodine load.
23 Non-thyroidal illness lowers Tโ‚ƒ first and raises reverse Tโ‚ƒ; avoid testing acutely.
24 PTU is preferred in the first trimester; radioiodine is contraindicated in pregnancy.
25 Levothyroxine reaches steady state in ~6 weeks โ€” recheck TSH no sooner.
๐Ÿ’ก The single most testable pattern
High total Tโ‚„ + normal TSH + normal free Tโ‚„ = a binding-protein (TBG) effect, NOT hyperthyroidism. The exam loves the pregnant or estrogen-using patient. Always anchor on TSH and free hormone, never total hormone alone.
