Section 1 โ€” Priority Map & Quick Foundation

Topic Priority Why It’s Tested
PSA (free %, velocity, recurrence cutoffs, DRE timing) HIGH Most-tested marker; gray zone, pre-analytical traps, post-op interpretation
AFP / hCG / LDH in germ cell tumors HIGH Prognosis & staging (IGCCCG); AFP excludes pure seminoma; half-lives
CA125 + HE4 / ROMA / RMI (ovarian) HIGH Menstruation & renal-failure pitfalls; MRC-EORTC monitoring lesson
CEA (colorectal) & K-RAS predictive testing HIGH 30% rise rule; K-RAS predicts anti-EGFR resistance
AFP in HCC (>200 ยตg/L rule, HBV/aflatoxin) HIGH Screening high-risk groups; near-diagnostic cutoff
Assay interferences (HAMA, hook effect, anti-Tg) HIGH Falsely high vs falsely low โ€” classic distractors
CA19-9 (pancreatic, Lewis-negative, cholestasis) MEDIUM Genetic non-producers; obstruction false positives
Breast markers (HER2, ER, CA15-3, uPA/PAI-1, Oncotype DX) MEDIUM Predictive vs prognostic vs monitoring roles
Thyroid (thyroglobulin vs calcitonin) MEDIUM Differentiated vs medullary; anti-Tg interference
Lung (NSE/ProGRP vs Cyfra 21-1; ALK, EGFR) MEDIUM SCLC vs NSCLC marker matching
Screening principles (sensitivity, specificity, PPV, prevalence) MEDIUM PPV falls as prevalence falls โ€” conceptual favorite
History (Bence Jones, RIA, oncofetal antigens) LOW One-liners only; rarely more than a single recall item
KEY POINT โ€” where to spend your time: Master the pre-analytical traps (DREโ†’PSA, menstruationโ†’CA125, cholestasisโ†’CA19-9), the marker-to-tumor pairings, the half-lives (hCG 1โ€“2 d, AFP 5โ€“6 d), and the predictive markers (ER, HER2, K-RAS, BRAF). Skim the historical trivia and deep molecular biology.

Quick Foundation โ€” start from zero

  • A tumor marker is a substance made by a tumor (or by the body reacting to it) that we can measure in blood, urine, or tissue.
  • Markers are mostly for monitoring treatment and detecting recurrence โ€” only a few are good enough to screen healthy people.
  • A good screening test needs high sensitivity AND high specificity, and even then it only works well when the disease is reasonably common in the group tested.
  • Oncofetal antigens (AFP, CEA, hCG) are normally fetal proteins that reappear in some adult cancers.
  • After curative surgery, a marker should fall by its half-life; a slow or rising marker means leftover tumor.
  • Most “wrong” exam answers are pre-analytical or assay artifacts, not real disease.
Analogy: Think of a tumor marker like a smoke detector. It tells you something may be burning (monitoring) but it also beeps at burnt toast (false positives from menstruation, cholestasis, renal failure). You confirm with the whole picture before calling the fire department.

Why this matters clinically: Acting on a single marker value โ€” without checking pre-analytical conditions and trends โ€” is the commonest cause of misdiagnosis the ASCP exam wants you to avoid.

Concept Plain-English Explanation
Sensitivity Of people WITH cancer, how many test positive (catches disease)
Specificity Of people WITHOUT cancer, how many test negative (avoids false alarms)
PPV If positive, chance it’s real โ€” shrinks when the disease is rare
Half-life Time for the marker to drop by 50% after the tumor is gone
Predictive marker Tells you if a specific drug will work (e.g., HER2 โ†’ trastuzumab)
Prognostic marker Tells you how aggressive the disease is, regardless of treatment

Section 2 โ€” Core Content Tables

PSA โ€” Prostate-Specific Antigen

Feature Details
Tissue source Prostate epithelium (a kallikrein serine protease)
Blood form Most immunoreactive PSA is complexed with ฮฑ1-antichymotrypsin (ACT); some is “free”
Gray zone Total PSA 4โ€“10 ยตg/L โ†’ use percent-free PSA to separate cancer from BPH
% free PSA <10% โ†’ >50% probability of cancer (cancer lowers the free fraction)
PSA velocity Rate of change of PSA over time
Recurrence โ€” surgery AUA: โ‰ฅ0.2 ยตg/L confirmed = biochemical recurrence
Recurrence โ€” radiotherapy Phoenix: rise of โ‰ฅ2.0 ยตg/L above nadir
Pre-analytical Draw before DRE or after a delay (DRE transiently raises PSA)

After prostatectomy the gland is gone, so the expected PSA is undetectable โ€” any confirmed rise (even within the population reference interval) signals residual/recurrent disease.

PRE-ANALYTICAL โ€” PSA: Serum/plasma; collect before DRE, prostate biopsy, or catheterization. Use the same method for serial monitoring (results aren’t interchangeable). Post-prostatectomy expectation = undetectable.

AFP, hCG, LDH โ€” Germ Cell Tumor Panel

Marker Half-life Key Exam Points
AFP 5โ€“6 days Increased AFP excludes pure seminoma; good-prognosis NSGCT needs AFP <1000 ยตg/L (830 kU/L)
hCG 1โ€“2 days Poor prognosis NSGCT if hCG >50,000 U/L; assay must detect intact dimer and free ฮฒ (hCGฮฒ)
LDH โ€” Indicator of total tumor burden, not a specific cell type

AFP, hCG, and LDH are mandatory for prognosis and chemotherapy selection in metastatic nonseminomatous germ cell tumors (IGCCCG staging).

PRE-ANALYTICAL โ€” hCG: Use an oncology hCG assay that recognizes intact + free ฮฒ-subunit. Very high hCG can cross-react with TSH receptors โ†’ hyperthyroidism. AFP is physiologically high in infants <6 months โ€” interpret with caution.

CA125 & HE4 โ€” Ovarian

Feature Details
CA125 โ€” best for Monitoring; most sensitive for serous adenocarcinoma
CA125 โ€” “response” โ‰ฅ50% reduction from pretreatment, confirmed/maintained โ‰ฅ28 days
HE4 Superior to CA125 for distinguishing malignant vs benign pelvic masses in premenopausal women
RMI Ultrasound score (U) ร— Menopausal status (M) ร— serum CA125
ROMA HE4 + CA125 + menopausal status
PRE-ANALYTICAL โ€” CA125 & HE4: Avoid CA125 during menstruation (transient false rise). HE4 rises in renal failure โ†’ false positives. Don’t act on a single CA125 rise (MRC-EORTC: no survival benefit, worse QoL).

CEA & CA19-9 โ€” GI

Marker Tumor Key Points
CEA Colorectal Significant rise = โ‰ฅ30% over prior; surrogate for CT in liver-met response; K-RAS must be tested before anti-EGFR (cetuximab)
CA19-9 Pancreatic ductal adenocarcinoma Undetectable in Lewis a-negative (5โ€“10%); measure only after biliary decompression
PRE-ANALYTICAL โ€” CA19-9: Biliary obstruction/cholangitis falsely raises CA19-9 โ€” decompress first. CEA can rise with smoking and benign inflammation.

AFP โ€” Hepatocellular Carcinoma

Feature Details
Screening AFP for high-risk groups (chronic HBV, cirrhosis)
Near-diagnostic AFP >200 ยตg/L (170 kU/L) + hypervascular liver lesion = virtually HCC
Risk factors Asia/Africa: chronic HBV + aflatoxin B

Breast, Thyroid & Lung โ€” Quick Pairings

Marker Role Use
ER (estrogen receptor) Predictive Antiestrogen therapy response
HER2 Predictive Identifies candidates for trastuzumab
CA15-3 / BR27.29 Monitoring Breast cancer marker of choice
uPA / PAI-1 Prognostic Best-validated in node-negative breast cancer
Oncotype DX (21 genes) Prognostic/predictive 10-yr distant recurrence risk
Thyroglobulin Monitoring Differentiated (papillary/follicular) thyroid cancer
Calcitonin Diagnosis/monitoring Medullary thyroid carcinoma
NSE / ProGRP Monitoring Small cell lung cancer (SCLC)
Cyfra 21-1 Monitoring Non-small cell lung cancer (NSCLC)
PRE-ANALYTICAL โ€” Thyroglobulin: Always run anti-thyroglobulin antibodies alongside; they cause falsely LOW Tg in immunometric assays, masking residual disease.

Section 3 โ€” Visual Memory Hooks

๐Ÿง  “SAL” for germ cell tumors โ€” Seminoma? AFP must be silent. AFP up โ†’ not pure seminoma. LDH = total tumor Load. (Markers: AFP, hCG, LDH.)
๐Ÿง  “Halves”: hCG Hurries, AFP Ambles โ€” hCG โ‰ˆ 1โ€“2 days, AFP โ‰ˆ 5โ€“6 days. Faster name, faster clearance.
๐Ÿง  “Free & Low = Lurking cancer” โ€” % free PSA <10% โ†’ >50% cancer probability. Cancer eats the free fraction.
๐Ÿง  “Don’t test on a Period or with a blocked Pipe” โ€” CA125 during menstruation and CA19-9 with biliary obstruction = false positives.
๐Ÿง  “HAMA High, Hook Low” โ€” HAMA = falsely HIGH; high-dose hook = falsely LOW in obvious disease.

Pattern Trigger Table

If you seeโ€ฆ Thinkโ€ฆ Becauseโ€ฆ
โ†‘AFP in a testicular tumor NOT pure seminoma Seminomas don’t make AFP
โ†‘PSA right after DRE Pre-analytical false rise DRE transiently elevates PSA
โ†‘CA19-9 in a jaundiced patient Confirm after decompression Cholestasis raises CA19-9
Undetectable CA19-9 despite tumor Lewis a-negative patient 5โ€“10% can’t synthesize it
Falsely LOW marker in advanced disease High-dose hook effect Antigen saturates both antibodies
Discordant HIGH marker after mouse-Ab imaging HAMA interference Antibodies bridge reagent Abs
Undetectable Tg + positive anti-Tg Unreliable (falsely low) Autoantibodies interfere with assay
โ†‘HE4 with kidney disease Possible false positive HE4 rises in renal failure

Commonly Confused โ€” Side by Side

Pair A B
Thyroid markers Thyroglobulin = differentiated Calcitonin = medullary
Lung markers NSE/ProGRP = SCLC Cyfra 21-1 = NSCLC
Ovarian indices RMI = US + menopause + CA125 ROMA = HE4 + CA125 + menopause
Interference direction HAMA = high Hook = low
Predictive markers HER2/ER = breast K-RAS = colorectal anti-EGFR

Diagnostic Window โ€” Marker Clearance

Marker Half-life Clears by ~ Best Clinical Use
hCG 1โ€“2 days 1โ€“2 weeks Fast post-op monitoring of GCT/trophoblastic disease
AFP 5โ€“6 days 4โ€“6 weeks Slower fall flags residual yolk-sac/HCC tumor

Section 4 โ€” Diagnostic Flowcharts

โ‘  Testicular mass โ€” choosing the diagnosis

Young man, testicular mass โ†’ draw AFP, hCG, LDH
โ†’ AFP elevated? โ†’ YES โ†’ nonseminomatous (pure seminoma excluded)
โ†’ AFP normal, hCG modest, “seminoma” on path โ†’ consistent with seminoma
โ†’ Use AFP/hCG/LDH for IGCCCG prognosis & chemo
โš ๏ธ Common mistake: Calling it “pure seminoma” despite an elevated AFP. Any AFP rise reclassifies the tumor.
โ‘ก Elevated PSA in the gray zone

Total PSA 4โ€“10 ยตg/L โ†’ was blood drawn before DRE/biopsy?
โ†’ NO โ†’ repeat on a clean specimen first
โ†’ YES โ†’ order % free PSA
โ†’ % free <10% โ†’ high cancer probability โ†’ urology/biopsy
โš ๏ธ Common mistake: Interpreting a post-DRE PSA as a true value and rushing to biopsy.
โ‘ข Pelvic mass โ€” malignant or benign? (method selection)

Adnexal mass โ†’ premenopausal?
โ†’ YES โ†’ HE4 discriminates better than CA125 (CA125 less specific here)
โ†’ Combine into ROMA (HE4+CA125+menopause) or RMI
โ†’ Renal failure present? โ†’ interpret HE4 cautiously (false โ†‘)
โš ๏ธ Common mistake: Trusting HE4 in a patient with chronic kidney disease.
โ‘ฃ Unexpected marker result โ€” assay artifact? (timing/interference)

Marker doesn’t fit the clinical picture โ†’ which direction?
โ†’ Falsely LOW in obvious advanced disease โ†’ re-assay at two dilutions (hook effect)
โ†’ Falsely HIGH, recent mouse-Ab imaging โ†’ add blocker / change method (HAMA)
โ†’ Undetectable Tg โ†’ check anti-Tg antibodies
โš ๏ธ Common mistake: Reporting a hooked low value as a “good treatment response.”
โ‘ค Liver lesion โ€” is it HCC?

High-risk patient (chronic HBV / cirrhosis) + hypervascular liver lesion โ†’ measure AFP
โ†’ AFP >200 ยตg/L (170 kU/L) โ†’ virtually diagnostic of HCC
โ†’ AFP mildly up only โ†’ correlate with imaging; consider benign liver disease
โš ๏ธ Common mistake: Dismissing AFP as “non-specific” and ignoring the lesion-plus-cutoff rule.

Section 5 โ€” 25 High-Yield Facts

# Fact
1 A tumor marker is a substance present in/produced by a tumor or produced by the host in response to a tumor.
2 Bence Jones protein (monoclonal Ig light chain) was the first clinically used tumor marker (1847).
3 Radioimmunoassay (RIA), developed in the 1960s, brought tumor markers into routine practice.
4 Oncofetal antigens (AFP, CEA, hCG) are fetal proteins re-expressed in some adult cancers.
5 As disease prevalence falls, positive predictive value (PPV) falls even if sensitivity/specificity are constant.
6 Sensitivity = TP/(TP+FN); Specificity = TN/(TN+FP).
7 An increased AFP precludes a diagnosis of pure seminoma.
8 AFP, hCG, and LDH are mandatory for prognosis/chemo selection in metastatic nonseminomatous germ cell tumors.
9 hCG half-life โ‰ˆ 1โ€“2 days; AFP half-life โ‰ˆ 5โ€“6 days.
10 hCG sensitivity/specificity for gestational trophoblastic neoplasia โ‰ˆ 99%.
11 Oncology hCG assays must detect both intact dimer and free ฮฒ-subunit (hCGฮฒ).
12 AFP >200 ยตg/L (170 kU/L) with a hypervascular liver lesion is virtually diagnostic of HCC.
13 Major HCC risk factors in Asia/Africa: chronic HBV infection and aflatoxin B.
14 CA125 is most sensitive for serous adenocarcinoma; avoid testing during menstruation.
15 HE4 beats CA125 for malignant-vs-benign pelvic masses in premenopausal women but rises in renal failure.
16 ROMA = HE4 + CA125 + menopausal status; RMI = ultrasound + menopause + CA125.
17 MRC-EORTC: treating ovarian recurrence on CA125 rise alone gave no survival benefit and worse quality of life.
18 PSA gray zone is 4โ€“10 ยตg/L; % free PSA <10% indicates >50% cancer probability.
19 Post-prostatectomy PSA should be undetectable; AUA recurrence = โ‰ฅ0.2 ยตg/L confirmed.
20 Radiotherapy (Phoenix) PSA recurrence = rise โ‰ฅ2.0 ยตg/L above nadir; draw PSA before DRE.
21 CA19-9 is undetectable in Lewis a-negative people (5โ€“10%) and falsely rises with biliary obstruction.
22 Colorectal CEA significant rise = โ‰ฅ30%; test K-RAS before anti-EGFR (cetuximab) โ€” mutation predicts resistance.
23 HAMA cause falsely HIGH results; high-dose hook effect causes falsely LOW results in very high-antigen samples.
24 Thyroglobulin monitors differentiated thyroid cancer (anti-Tg causes falsely low); calcitonin marks medullary carcinoma.
25 SCLC โ†’ NSE/ProGRP; NSCLC โ†’ Cyfra 21-1; HER2 โ†’ trastuzumab; ER โ†’ antiestrogen therapy; BRAF V600E โ†’ vemurafenib (melanoma).
EXAM TIP โ€” the single most testable pattern: When a marker value doesn’t match the patient, suspect a pre-analytical or assay artifact first (DREโ†’PSA, mensesโ†’CA125, cholestasisโ†’CA19-9, HAMA=high, hook=low, anti-Tg=low) before calling it true disease.
