๐ŸŽฏ KEY POINT โ€” Where to spend your time

Focus: every formula in this chapter (hemacytometer, NRBC correction, indices, retic %, ARC, corrected retic, RPI) and the error lists (cyanmethemoglobin, microhematocrit, ESR). Skim: brand names, POCT program administration details.

Quick Foundation โ€” from zero

  • Analyzers do most counting, but manual methods remain the backup (instrument down, counts beyond linearity, remote or disaster settings) and a QC check.
  • A hemacytometer is a glass chamber with a ruled grid of known area and depth; counting cells in a known volume of a known dilution gives cells per ยตL.
  • Hemoglobin is measured by converting all of it to one colored form (cyanmethemoglobin) and reading absorbance at 540 nm.
  • The hematocrit is the packed red cell volume after centrifugation. Hb, Hct and RBC count together give the indices (MCV, MCH, MCHC).
  • The reticulocyte count shows whether the marrow is responding to anemia; corrections adjust it for anemia and for early-released (shift) cells.
  • The ESR is a nonspecific marker of inflammation based on how fast RBCs settle.
๐Ÿ’ก Analogy

A hemacytometer is like counting people in one marked tile of a stadium floor and multiplying up: you need the tile size (area), ceiling height (depth 0.1 mm) and how much you watered down the crowd (dilution). The reticulocyte corrections are like judging a factory: a raw count of new cars looks impressive if the parking lot is half empty (anemia โ†’ corrected retic), and cars that stay “new” longer because they left the factory early must not be double-counted (shift cells โ†’ RPI).

๐Ÿฅ Why this matters clinically

A lipemic sample can falsely raise Hb and hide an anemia; the rule of three catches it. Point-of-care hematocrits read by conductivity can be falsely low in patients with abnormal electrolytes or low protein โ€” common in ICU and bypass patients. The RPI separates a marrow that is responding (hemolysis, bleeding) from one that is failing.

Concept Plain-English Explanation
Improved Neubauer ruling 3 ร— 3 mm grid of nine 1-mmยฒ squares, 0.1 mm deep
Dilution factor Reciprocal of the dilution (1:20 โ†’ 20)
Cyanmethemoglobin Stable Hb derivative read at 540 nm โ€” reference method
PCV Packed cell volume = hematocrit
Rule of three Hb ร— 3 โ‰ˆ Hct (ยฑ 3) for normocytic normochromic cells
ARC Absolute reticulocyte count = retic % ร— RBC count
Shift reticulocytes Retics released early that stay immature 2โ€“3 days in blood
RPI Retic count corrected for anemia and maturation time
ESR Millimeters RBCs fall in 1 hour
Waived test Simple test with insignificant risk of an erroneous result (CLIA)

2 ยท Core Content Tables

5 ยท 25 High-Yield Facts

# Fact
1 The improved Neubauer hemacytometer grid is 3 ร— 3 mm (9 mmยฒ) with a depth of 0.1 mm.
2 Each small square of the center square is 0.2 ร— 0.2 mm (0.04 mmยฒ).
3 Cell count = cells counted ร— dilution factor รท (area in mmยฒ ร— 0.1 mm depth).
4 Manual WBC counts use a 1:20 dilution with an RBC-lysing diluent and the four corner squares (4 mmยฒ).
5 Both sides of the chamber must agree within 10%.
6 Cells touching the top and left lines are counted; those touching the bottom and right are not.
7 NRBCs are counted as WBCs in manual counts; correct when โ‰ฅ5 NRBCs per 100 WBCs: WBC ร— 100 รท (NRBC + 100).
8 Manual platelet counts use 1:100 ammonium oxalate, phase contrast at 40ร—, and the 1-mmยฒ center square.
9 Cyanmethemoglobin is the CLSI reference method for hemoglobin, read at 540 nm.
10 Ferricyanide oxidizes Feยฒโบ to Feยณโบ (methemoglobin); cyanide converts it to cyanmethemoglobin.
11 Sulfhemoglobin is not measured by the cyanmethemoglobin method.
12 Lipemia, WBCs >20 ร— 10โน/L, platelets >700 ร— 10โน/L, Hb S/C and abnormal globulins cause turbidity and falsely high hemoglobin.
13 Duplicate microhematocrits must agree within 1%, and the buffy coat is excluded.
14 Excess anticoagulant (short draw) falsely lowers the microhematocrit; insufficient centrifugation raises it.
15 Trapped plasma raises the microhematocrit 1โ€“3% in sickle cell, spherocytosis, thalassemia and macrocytic or hypochromic anemias.
16 The rule of three: Hb ร— 3 = Hct ยฑ 3, valid only for normocytic, normochromic cells.
17 MCV = Hct ร— 10 รท RBC; MCH = Hb ร— 10 รท RBC; MCHC = Hb ร— 100 รท Hct.
18 An MCHC of 36โ€“38 g/dL should prompt a search for spherocytes; above 38 suggests an Hb error or cold agglutinin.
19 A reticulocyte has two or more blue-staining particles with new methylene blue.
20 With a Miller disc, count at least 112 RBCs in the small square, which is 1/9 of the large square.
21 Corrected reticulocyte count = retic % ร— patient Hct รท 45.
22 RPI = corrected retic รท maturation time; RPI >3 is an adequate response and <2 inadequate.
23 The ESR depends on rouleaux formation promoted by fibrinogen and globulins.
24 The modified Westergren method (200-mm column, 4:1 dilution) is the ICSH/CLSI-recommended ESR method.
25 CLIA test site neutrality means all testing sites follow the same rules based on test complexity.
โญ EXAM TIP โ€” The single most testable pattern

Know the formulas cold: count = cells ร— dilution รท (area ร— 0.1); corrected WBC = WBC ร— 100 รท (NRBC + 100); MCV = Hct ร— 10 รท RBC; MCHC = Hb ร— 100 รท Hct; corrected retic = retic ร— Hct/45; RPI = corrected รท maturation. Turbidity โ†’ falsely high Hb. Sickle cells/spherocytes/polycythemia โ†’ low ESR.
